About the Position
The successful candidate will provide comprehensive support services to families struggling with substance abuse, utilizing a Solution-Based Casework approach. This involves engaging clients in treatment planning, collaborating with multidisciplinary teams, and facilitating recovery processes. This role offers a competitive salary range of $60,000.00 to $63,000.00 annually, plus potential performance-based merit awards and a comprehensive benefits package. About Graham Windham's FTR Program
The Family Treatment/Rehabilitation Program serves families at risk of removal due to neglect and abuse. Our goal is to partner with families, ensuring their safety while promoting positive change. We focus on engagement, service delivery, and collaboration, building connections with supportive resources to empower families to sustain progress.

Your Key Responsibilities
- Provide case management, substance abuse counseling, and advocacy services
- Participate in interdisciplinary treatment teams, facilitating client recovery and rehabilitation
- Refer and broker targeted interventions as needed, contributing to a healthy program environment
- Consult with case planners regarding families struggling with substance abuse
- Conduct intakes, including psychosocial history, and design individual treatment plans
- Coordinate consumer treatment with other organizations, as indicated
- Establish professional relationships and serve as a positive role model
- Maintain accurate records, update treatment plans, and prepare required reports
