Job Summary

Vynca is seeking a highly skilled Lead Care Manager to join our team in Santa Clara, CA. As a key member of our care management team, you will be responsible for providing exceptional care coordination services to our clients.

Key Responsibilities
  • Assess client needs in areas such as physical health, mental health, social determinants, and referral to community-based services.
  • Develop and implement comprehensive care plans, setting SMART goals and evaluating client progress.
  • Provide education and support to clients and their caregivers, promoting wellness, recovery, and independence.
  • Collaborate with healthcare professionals, social services providers, and other stakeholders to ensure seamless care coordination.
  • Utilize evidence-based practices, such as Motivational Interviewing and Trauma-Informed Care, to support client needs.
  • Conduct outreach and engagement activities to facilitate linkage to our care management program.
  • Document client progress and maintain accurate records in our Electronic Medical Record (EMR) system.
  • Complete monthly reporting to ensure program compliance.
Requirements
  • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
  • Working knowledge of government and community resources related to social determinants of health.
  • Clean driving record, valid driver's license, and reliable transportation.
  • Excellent oral and written communication skills.
  • Positive interpersonal skills required.
  • General computer skills and working knowledge of Google Workspace, MS Office, and the internet.
What We Offer
  • $26-30 hourly rate, based on experience and qualifications.

Essential Organizational Duties
  • Work within company policies and guidelines at all times.
  • Use video conferencing frequently.
  • Ensure effective use of resources.
  • Promote good relationships with all stakeholders.
  • Respect confidentiality and maintain HIPAA compliance.
