Job Description

Complete Staffing Solutions is seeking a skilled professional to fill the role of Care Coordinator. This position will be responsible for providing case management to formerly homeless individuals and families, with a focus on maintaining stable housing in scattered site or community-based locations.

The ideal candidate will have a Bachelor's degree in a relevant field, along with one year of related work experience in homelessness, recovery supports, harm reduction, or trauma-informed service delivery. Alternatively, we will consider applicants with 5+ years of experience specifically related to families exiting homelessness, particularly those with a history of substance use, even without a degree.

The successful candidate will also possess a valid driver's license and access to a car for travel between client locations and various community resources. They will provide evidence-based practices, such as Motivational Interviewing, to assist clients in developing and monitoring individual service plans (ISP). Additionally, they will help clients access resources and services to address housing barriers, including CORI mitigation, credit repair, and financial applications.
