Clinical Pharmacist

Dartmouth Health seeks a highly skilled Clinical Pharmacist to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the safe and effective use of medications across our facilities.
Key Responsibilities- Gather and reconcile medication histories to identify potential issues and ensure patient safety.
- Conduct comprehensive medication reviews to make evidence-based recommendations for optimal medication use.
- Identify and report adverse drug reactions, adherence gaps, and other safety concerns.
- Collaborate with healthcare teams to optimize medication therapy and improve patient outcomes.
- Provide education and consultation to patients, caregivers, and healthcare professionals on medication use and safety.
- Participate in quality improvement initiatives to enhance medication use and patient safety.
- Graduate of an ACPE-accredited School of Pharmacy with a Bachelor's of Science in Pharmacy.
- Doctor of Pharmacy (Pharm D) degree preferred.
- Completion of a post-graduate residency and/or board certification in a relevant area is a plus.
- Current licensure to practice pharmacy in the state of NH and ability to work independently with minimal supervision.
- Excellent communication and interpersonal skills.
