**Job Title:** Clinical Nurse I- D4E: Hematology/Oncology

About the Role

We are seeking a highly skilled Clinical Nurse to join our team in Hematology/Oncology at Albany Medical Center. As a key member of our caregiving team, you will be responsible for providing exceptional patient care and ensuring seamless transitions within the acute care stay.

Key Responsibilities
  • Patient Care Coordination: Assess, plan, direct, implement, and manage the needs of assigned patients throughout their episode of care.
  • Team Collaboration: Coordinate the delivery and coordination of patient care provided by the caregiving team, ensuring competent and compassionate care is uniformly provided.
  • Communication: Effectively communicate with referring facilities, hospital staff, and patients in various settings, including inpatient, ambulatory, and community environments.
