Deliver Compassionate Home Health Care as a Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse to provide high-quality care to patients in their homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.


About the Role:

This full-time position requires one year of clinical experience and one year of home health experience. You will be responsible for providing nursing visits according to the treatment prescribed by the physician and maintaining accurate and complete medical records.


Key Responsibilities:
  • Provide comprehensive patient care in home health settings
  • Conduct thorough assessments and develop individualized care plans
  • Maintain accurate and up-to-date medical records
  • Collaborate with healthcare teams to ensure seamless care delivery

Requirements:
  • Graduate of an accredited practical or vocational nursing program
  • Currently licensed as a Licensed Practical Nurse in the state of operation
  • Valid CPR certification
  • Licensed driver able to travel to all business locations

What We Offer:
