Job Summary:
Always Best Care Home Health is seeking a skilled RN Clinical Manager to lead our home health team. As a key member of our leadership team, you will be responsible for guiding and managing our clinical staff to deliver high-quality patient care.

Key Responsibilities:
  • Develop and implement nursing services, objectives, policies, and procedures in accordance with Federal and State regulations.
  • Coordinate, organize, implement, and evaluate the patient care delivery system.
  • Monitor clinical documentation for completeness and accuracy.
  • Provide education to clinical staff and patients as needed.
  • Meet with physicians, social workers, and other healthcare professionals to monitor patient status and effectiveness of care plans.
  • Identify issues and direct changes to ensure efficient, cost-effective, and appropriate care.
  • Collect and analyze data to inform decision-making and improve patient outcomes.
  • Monitor financial data to ensure the office is operating at peak efficiency.

Requirements:
  • RN with current license to practice in California.
  • Minimum of five years of nursing experience, with a preference for at least one year in home care.
  • Knowledge of community resources, Medicare, and state regulations.
  • Excellent communication, interpersonal, and documentation skills.
  • Demonstrated organizational and time management skills, critical thinking, and ability to work independently.
  • Computer data entry skills.

Preferred Qualifications:
  • At least three to five years of experience in a Medicare Home Health environment.
  • Proven track record of leading and managing teams with at least two to three years in a leadership role.
  • Outstanding desire to provide great customer service and creative clinical management.
