Key Responsibilities
  • Assess patient health problems and needs, developing and implementing individualized nursing care plans.
  • Deliver high-quality, patient-centered care, including medication management, wound care, and vital sign monitoring.
  • Collaborate with interdisciplinary teams to coordinate patient care and address complex medical needs.
  • Maintain accurate and comprehensive medical records, documenting patient progress and outcomes.
Requirements
  • Associate degree required; Bachelor's degree preferred.
  • Current California Registered Nurse License.
  • Prior experience in home health care or a related field.
  • Current CPR certification.
  • Strong interpersonal and communication skills.
  • Computer literacy and proficiency in electronic medical records.
  • Excellent organizational and time management skills.
Estimated Salary: $90,000 - $110,000 per year
