Job Summary
We are seeking a skilled and empathetic Registered Nurse to join our team as a Hospice RN/ RN Case Manager. In this role, you will provide direct patient care services, support patients and their families, and collaborate with an interdisciplinary team to deliver high-quality care.
Key Responsibilities
- Direct Patient Care
- Provide skilled nursing care to patients in their homes, hospitals, or other care settings.
- Assess patient needs and develop individualized care plans.
- Implement and evaluate the effectiveness of care plans.
- Interdisciplinary Collaboration
- Work closely with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive care.
- Participate in patient care conferences and contribute to the development of care plans.
- Family Support
- Provide emotional support and education to patients and their families.
- Assist patients and families in navigating the healthcare system.
- Documentation and Reporting
- Maintain accurate and timely documentation of patient care.
- Prepare and submit reports as required.
Requirements
- Education and Licensure
- Current RN license in the state of employment.
- Bachelor's degree in nursing or related field.
- Experience
- At least one year of experience in hospice, home health, ICU, geriatric, or oncology nursing.
- Experience working with patients with terminal illnesses and their families.
- Skills and Abilities
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
