Hospice Care Specialist

About the Role
">At Gentiva Health Services, we are committed to providing compassionate care to those in need. As a Hospice Aide/ Certified Nursing Assistant, you will play a vital role in delivering high-quality care to our patients and their families.
">About You

- You have a strong passion for providing care and support to individuals in need.">
- You possess a current Certified Nursing Assistant (CNA) license.">
- You have at least six months of patient care experience in hospice, home health, ICU, geriatric, or oncology.">
- You are comfortable working in a fast-paced environment and are able to adapt to changing situations.">
Responsibilities
">- ">
- Report directly to the Administrator, Executive Director or Patient Care Manager Senior and function under the direction, instruction and supervision of the Registered Nurse and/or Clinical Supervisor.">
- Help the patient to maintain good personal hygiene.">
- Assist in maintaining a safe and healthy environment.">
- Prepare and serve light meals.">
- Assist in feeding patient when needed.">
- Assist the patient in ambulation.">
- Assist with the safe use of a walker or wheelchair as taught and supervised by the nurse.">
- Assist with light housekeeping tasks such as emptying trash and changing linens.">
- Assist with certain treatments as ordered on the aide plan of care and as allowable in state-specific regulations.">
- Attempt to promote patient's mental alertness through involvement in activities of interest.">
- Give basic emotional and psychological support to the patient and other members of the household.">
- Report any change in the patient's mental or physical condition or home situation to the Registered Nurse and members of the interdisciplinary team.">
- Demonstrate basic care to the primary caregiver.">
- Practice infection control measures and standard precautions.">
- Provide accurate documentation with visit itineraries">
- Attend staff meetings.">
- Participate in at least one Quality Improvement activity yearly, as assigned by supervisor.">
- Complete documentation in a timely manner as outlined in orientation.">
