Job Title: Clinical Case Manager

Job Summary:

Bonita House, Inc. is seeking a highly skilled Clinical Case Manager to join our team. As a Clinical Case Manager, you will be responsible for overseeing the clinical needs of our clients, providing supportive services, and facilitating their progress towards self-sufficiency and recovery.

Key Responsibilities:

  • Coordinate with the clinical team to provide supportive services to 20-25 adults referred from CalWORKs Welfare-to-Work agency.
  • Facilitate weekly/monthly group activities that promote self-sufficiency, recovery, wellness, and healthy boundaries.
  • Provide psychoeducation to clients and family members on health, well-being, and lifestyle, including symptom management, medication, nutritional, sleep hygiene, trauma-informed care, and relapse prevention.
  • Complete initial bio-psychosocial assessments, annual assessments, and establish collaborative treatment goals.
  • Link clients to housing, employment, medical, dental, psychological, parenting, legal, transportation, and money management resources.
  • Manage and resolve crisis situations effectively.
  • Attend all clinical supervision, case conferences, and team meetings as scheduled.

Requirements:

  • Active registration with BBS, Registered MFT or ASW.
  • Ability to work independently and as a member of a cohesive team.
  • Effective prioritization and time management skills.
  • Strong written and interpersonal skills, including emotional intelligence.
  • Ability to meet Immigration Reform Act of 1986 requirements.
  • Valid California Motor Vehicle Operator's License and automobile insurance.

Preferred Qualifications:

  • Bilingual (Spanish) preferred.
  • Previous experience working in an Electronic Health Record (EHR).

Physical Requirements:

The position requires frequent communication with others, including hearing and speaking clearly with individuals and groups, in person and via phone and video calls. Ability to lift 50 pounds at times. Regular local travel and operation of a motor vehicle is required.
