Key Responsibilities

Essential Duties include:

  • Ongoing Resident Evaluation: Regularly assess residents (Medicare/Medicaid) to verify the achievement of care goals and continued eligibility for services.
  • Admissions Screening: Evaluate all prospective admissions for Medicare/Medicaid eligibility.
  • Care Plan Meetings: Organize and facilitate initial care plan meetings for new residents.
  • Collaboration with Departments: Work alongside the Business Office and Rehabilitation Department to confirm reimbursements from Medicare, Medicaid, VA, or other funding sources.
  • Interdisciplinary Team Leadership: Act as the chairperson for the interdisciplinary assessment and care planning team, ensuring clear communication of goals and plans to residents and their families.
  • Coordination of Skilled Services: Collaborate closely with nursing and therapy teams to manage Medicare skilled services effectively.
  • Healthcare Staff Meetings: Conduct weekly discussions with healthcare staff regarding rehabilitation residents to monitor progress and plan for safe discharges.
  • Medical Records Accuracy: Ensure the precision of medical records and coding practices, preparing timely reports for regulatory bodies and nursing leadership.
  • MDS Data Submission: Submit MDS data to the CMS website promptly as required.
  • Quality Indicator Reporting: Prepare quarterly quality indicator reports for nursing leadership using CMS data to identify alerts and necessary actions.
  • Regulatory Compliance: Maintain current knowledge of federal and state regulations, particularly those related to Medicare and Medicaid programs.
  • Adherence to Standards: Ensure compliance with all relevant regulations, including DHEC, CMS, Fire and Safety, OSHA, and Labor Laws, while upholding HIPAA confidentiality standards.
  • Additional Duties: Perform other responsibilities as assigned.
