Job Summary:The LVN Charge Nurse will play a vital role in ensuring the delivery of high-quality care to our residents. This is a challenging and rewarding position that requires strong leadership and communication skills, as well as a commitment to providing exceptional patient care.
Responsibilities:- Assist in the development and implementation of individualized care plans for residents
- Conduct regular rounds to assess resident condition and identify areas for improvement
- Administer medications as prescribed and monitor resident response
- Collaborate with the healthcare team to ensure seamless care transitions
- Oversee and monitor the work of certified nursing assistants (CNAs) and other support staff
- Initiate and complete incident reports as needed
- Communicate effectively with residents, families, and healthcare team members
- Active Licensed Vocational Nurse (LVN) license in the state of Texas
- Experience working in a skilled nursing facility preferred
- Strong leadership and communication skills
- Ability to work effectively in a fast-paced environment
