Responsibilities- Provide personalized care and support to clients, including medication management, wound care, and mobility assistance.
- Develop and implement individualized care plans, working closely with clients, families, and healthcare teams to ensure seamless care coordination.
- Conduct thorough assessments and evaluations to identify client needs and develop effective care strategies.
- Collaborate with BrightStar Care's interdisciplinary team to ensure comprehensive care and support services.
- Maintain accurate and detailed records of client care, including progress notes and treatment plans.
- Current unrestricted Registered Nurse or Licensed Practical Nurse license in the state for which care is being provided.
- Minimum of 6 months of documented experience in home healthcare or a related field.
- Certified CPR/BLS and negative TB skin test or chest x-ray.
- Ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull, with varied assistance required depending on client mobility.
- Ability to lift up to 50 pounds.
- Competitive hourly rate of $41.00.
