About the Role

We are seeking a skilled Care Transition Specialist - RN to join our team at Lake Sunapee Region VNA & Hospice. As a key member of our organization, you will play a vital role in maintaining collaborative relationships between our agency and referral sources, hospitals, facilities, and provider offices.

Key Responsibilities
  • Maintain collaborative relationships with referral sources, hospitals, facilities, and provider offices to ensure seamless transitions of patients into our care.
  • Work closely with referral sources and our intake department to ensure a smooth transition of patients into our care.
  • Present educational programs to the community, referral sources, skilled nursing, and assisted living facilities as required.
  • Collaborate with our Home Care, Palliative, and Hospice clinical teams to ensure patients receive all beneficial services.
  • Conduct informational conversations with patients and families about palliative and hospice services.
  • Travel throughout our service area to meet with referral sources, patients, and families.
Requirements
  • Current NH RN license required.
  • Healthcare experience in Homecare, Hospice, and/or Case Management.
  • Affinity for networking and comfortable presenting to individuals and large groups.
  • Excellent written and oral communication skills.
