Job Summary

Community Access Inc is seeking a Peer Bridger to join our team in supporting Adult Home Residents in their transition to Supported Housing. The ideal candidate will have a strong understanding of recovery-oriented services and be able to build mentoring and supportive relationships with residents.

Key Responsibilities
  • Develop and implement individualized and group-based peer-informed strategies to promote housing transitions.
  • Engage residents in peer-based services to identify and overcome barriers to housing transition.
  • Build self-advocacy and systems navigation skills in residents.
  • Collaborate with community partners to provide resources and support to residents.
  • Participate in regular team meetings and contribute to the development of program goals and objectives.
Requirements
  • High school diploma or equivalent required.
  • New York Peer Specialist Certification and/or Howie the Harp graduate preferred.
  • Minimum of two years of experience in behavioral health settings, preferably in peer service delivery, community development, rehabilitation, or recovery-oriented services.
  • Recipient/survivor of mental health services required.
  • Strong understanding and practice of recovery, wellness, and related services.
  • Strong oral and written communication skills.
  • Ability to utilize various computer programs, specifically Microsoft Word and Excel.
  • Ability to travel and work afternoons, evenings, weekends, and holidays.
Preferred Qualifications
  • Bilingual candidates are encouraged to apply.
  • Personal or professional experience with New York City mental health supported housing.
