We are seeking a skilled and compassionate Social Services Assistant to join our team at Country Crest Post Acute. As a key member of our Social Services Department, you will play a vital role in ensuring the emotional and social well-being of our residents.

Responsibilities
  • Assist the Director of Social Services in planning, organizing, and implementing social services programs.
  • Coordinate transportation and appointments for residents, ensuring timely and efficient care.
  • Provide residents and families with accurate and timely information regarding Medicare, Medicaid, and financial assistance.
  • Maintain accurate social service progress notes and update the social services directory.
  • Collaborate with community social health and welfare agencies to meet the needs of our residents.
  • Protect resident confidentiality and maintain HIPAA standards.
Qualifications
  • High school graduate or equivalent.
  • Previous experience in skilled nursing preferred.
  • Strong communication and multitasking skills.
  • Ability to read, write, speak, and understand the English language.
  • CNA certification preferred.
  • Valid California Driver's license in good standing.
