Job Summary

We are seeking a skilled Home Health Physical Therapist to join our team at HCA Florida Healthcare at Home. As a key member of our patient care team, you will be responsible for providing high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Conduct thorough evaluations and assessments of patients to identify their physical therapy needs and develop individualized care plans.
  • Provide skilled therapeutic care to patients in their homes, using evidence-based practices and techniques to promote optimal outcomes.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Monitor patient progress, adjust treatment plans as needed, and document outcomes to ensure high-quality care.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to maintain expertise and provide exceptional care.
Requirements
  • Bachelor of Science in Physical Therapy from an accredited program.
  • Certifications, Licenses, and other Special Requirements.
  • Completion of the National Certification Examination for Registered Physical Therapist.
  • License to practice physical therapy within the state.
  • Ability to work varying hours, including weekends and holidays.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
