Rehabilitation Nurse

The Rehabilitation Registered Nurse at Amergis plays a vital role in assisting patients in achieving and maintaining optimal function due to chronic illness and/or debilitating injury.
The ideal candidate will be responsible for creating a recovery plan with the ultimate goal of achieving an independent lifestyle as much as possible.
Key Responsibilities:
- Assess the physical, psychological, sociocultural, and spiritual dimensions of clients and their families, as well as their educational and discharge needs.
- Implement a plan of care by providing nursing care and education directly or through ancillary personnel.
- Coordinate educational activities and develop and implement an individualized teaching and discharge plan with clients and their families.
- Perform hands-on nursing care by utilizing the nursing process to achieve quality outcomes for clients.
- Provide direction and supervision of ancillary nursing personnel and demonstrate professional judgment, using problem-solving techniques and delegating appropriately.
- Coordinate nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate the achievement of overall goals.
- Evaluate the nursing care that is being provided and modify the plan, as needed, to achieve measurable goals and objectives.
Requirements:
- Current Registered Nurse License for the state in which the nurse practices.
- One year prior professional nursing experience in a rehabilitation setting preferred.
- CRRN certification may be required per state/client contract.
- Complies with all relevant professional standards of practice.
- Participation and completion of Amergis' Competency program when applicable.
- Current CPR if applicable.
- TB questionnaire, PPD or chest x-ray if applicable.
- Current Health certificate (per contract or state regulation).
- Must meet all federal, state and local requirements.
- Successful completion of new hire training as applicable to job site.
- Understand patient confidentiality and HIPAA requirements.
- Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required.
- Computer proficiency required.
- Must be at least 18 years of age.
