Job Summary:

We are seeking a highly skilled and compassionate Care Manager to develop, assess, and facilitate complex care management activities for our members. As a Care Manager, you will work closely with healthcare providers, specialists, and community resources to provide high-quality, cost-effective healthcare outcomes.

Key Responsibilities:
  • Evaluate member needs, barriers to care, and social determinants of health to develop personalized care plans.
  • Collaborate with providers to identify resources and specialists to address member needs.
  • Identify problems and provide care management interventions to ensure timely access to needed care or services.
  • Provide ongoing follow-up and monitoring of member status, collaborating with members, caregivers, and providers to revise or update care plans as necessary.
  • Support members and care managers with local resources for various services, such as employment, housing, and independent living.
  • Facilitate care management and collaborate with providers to ensure timely access to needed care or services.
  • Collect, document, and maintain member information and care management activities to ensure compliance with state, federal, and third-party payer regulators.
  • Provide education to members and their families/caregivers on disease processes, resolving care gaps, and healthcare benefits.
  • Provide feedback to leadership on opportunities to improve and enhance care and quality delivery for members.
Requirements:
  • Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing and 2-4 years of related experience.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required.

Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law. Total compensation may also include additional forms of incentives.
