Job Summary

We are seeking a highly skilled RN Care Manager Field Specialist to join our team at UnitedHealth Group. In this role, you will be responsible for ensuring the continuity of care in the outpatient setting, utilizing advanced nursing knowledge and skills to ensure appropriate utilization of resources and patient quality outcomes.

About the Role

This is a full-time position that requires minimal supervision, and you will work independently with a high level of autonomy. You will be responsible for facilitating the continuum of patients' care, prioritizing patient care needs, and addressing emerging issues. You will also work closely with the care team, PCP, and other healthcare professionals to develop and implement effective care plans.

Key Responsibilities
  • Prioritize patient care needs upon initial visit and address emerging issues.
  • Meet with patients, patients' family, and caregivers as needed to discuss care and treatment plan.
  • Identify and assist with the follow-up of high-risk patients in acute care settings, skilled nursing facilities, custodial, and ambulatory settings.
  • Consult with physician and other team members to ensure that care plan is successfully implemented.
  • Coordinate treatment plans with the care team and triage interventions appropriate to the skill set of the team members.
Requirements

To be successful in this role, you must have:

  • Graduation from an accredited school of nursing.
  • Oregon State Registered Nurse license.
  • BLS certification (AHA) or CPR/AED certification (American Red Cross).
  • At least 3 years of experience in a clinical setting.
  • Oregon State driver's license and vehicle for work-related travel.
Preferred Qualifications

While not required, having the following qualifications would be beneficial:

  • Bachelor of Science in Nursing, BSN.
  • Telehealth certification.
  • Experience working in acute care.
  • Care management, utilization review, or discharge planning experience.
  • HMO experience.
Compensation

The estimated salary range for this role is $75,000 - $90,000 per year, depending on experience and location.
