Job Overview

We are seeking an experienced Licensed Vocational Nurse (LVN) to join our team as an End of Life Care Specialist. This role will involve providing direct patient care, implementing current nursing practices, and participating in the coordination of hospice services.

Responsibilities:
  • Provide direct patient care, including specialized duties related to end-of-life care.
  • Implement current nursing practice following comprehensive assessments and care plans.
  • Document patient services accurately and timely.
  • Educate patients and families on diagnosis and self-care plans.
  • Participate in interdisciplinary teams to coordinate hospice services.
  • Use equipment and supplies effectively, adhering to aseptic technique.
  • Manage pain and symptoms effectively.
  • Evaluate patient responses to treatments and medications.
Requirements:
  • Licensed LVN in the state with at least three years of nursing experience, preferably in community health or hospice.
  • Understanding of hospice philosophy and needs of terminally ill patients.
  • Flexibility in work hours.
  • Ability to maintain working knowledge of information systems and software programs.
