Job Description

We are seeking a skilled and compassionate Hospice and Palliative Care Nurse Practitioner to join our team at North Ottawa Community Health System. As a vital member of our healthcare team, you will provide high-quality patient care and support to individuals with complex medical needs.

Job Summary:

The Hospice and Palliative Care Nurse Practitioner is responsible for delivering comprehensive care to patients in their homes, hospitals, and hospices. This role involves assessing patient needs, developing personalized care plans, and collaborating with healthcare professionals to ensure seamless care delivery.

Key Responsibilities:
  • Clinical Responsibilities:
    • Conduct thorough assessments of patients' physical and emotional needs.
    • Develop and implement individualized care plans that address patients' goals and preferences.
    • Collaborate with healthcare teams to ensure timely and effective care delivery.
    • Make home visits as needed to provide ongoing care and support.
    • Complete documentation and reports in a timely manner to meet regulatory requirements.
  • Leadership:
    • Demonstrate accountability for personal, professional, and ethical standards.
    • Assist in the development and review of clinical protocols to ensure best practices.
    • Promote open communication across the continuum of care.
    • Act as a liaison between healthcare professionals and patients to ensure smooth care transitions.
  • Professional Development:
    • Stay current with the latest developments in Hospice and Palliative Care through ongoing education and training.
    • Participate in continuing education programs to enhance expertise and stay up-to-date with industry standards.
  • Performance Improvement:
    • Contribute to quality improvement initiatives to enhance patient outcomes and satisfaction.
    • Collaborate with the Quality and Compliance team to ensure adherence to regulatory requirements.

Requirements:
  • RN with Michigan license and Master's degree in Nursing required.
  • Current BLS certification and valid Michigan driver's license required.
  • Experience in Internal Medicine or Family Practice preferred, with Hospice and/or Geriatric experience a plus.
  • Certification in Hospice and Palliative Care preferred or willingness to obtain within 3 years.
  • Ability to lift 50 pounds, climb multiple stairs, and sit/stand/drive for extended periods.
  • Excellent communication and interpersonal skills, with ability to work effectively under pressure.
