Job Overview

About the Role

The successful candidate will provide holistic care to terminally ill patients and their loved ones, assessing their physical, psychological, social, and spiritual needs. They will work closely with an interdisciplinary team to develop individualized plans of care, monitor progress, and implement interventions to meet patient goals.

Responsibilities
  • Assessing patient needs and developing comprehensive care plans
  • Collaborating with physicians, hospice medical directors, and other healthcare professionals to ensure high-quality patient care
  • Coordinating patient services, including medications, equipment, and supplies
  • Educating patients, families, caregivers, and community members about hospice services and end-of-life care
Requirements
  • Current Registered Nurse degree required; hospice, home care, case management, or acute care experience preferred
  • Ability to work independently and as part of a team
  • Flexibility to adapt to changing patient needs, including occasional nights, weekends, and holiday on-call responsibilities
