Ambulatory Nurse Specialist Opportunity
Community Health Network is seeking an experienced Ambulatory Nurse Specialist to join our Family Medicine Care team. As a key member of our team, you will be responsible for conducting phone triage, patient education, scheduling appointments, referrals, prescription refills, and care coordination.
Key Responsibilities:
- Conduct phone triage and provide patient education
- Schedule appointments and referrals
- Process prescription refills and care coordination
- Collaborate with the healthcare team to ensure seamless patient care
Requirements:
- Graduate of a National League for Nursing (NLN) accredited school of nursing
- Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana
- One year or more of nursing experience required
- Previous medical office/outpatient experience preferred
