  • Work Schedule:

Compensation:

Key Responsibilities of the Nursing Team Leader:
  • Manage internal case management and develop nursing talent
  • Facilitate patient transitions from facilities to home through assessments and evaluations
  • Review and update care plans and documentation as needed
  • Assist in the recruitment, training, and validation of new nursing staff
  • Enhance the clinical skills of staff to ensure safe and quality care
  • Collaborate with operational and business development teams
  • Ensure compliance with all relevant policies and regulations
Qualifications for the Nursing Team Leader:
  • Must hold a valid RN license in the state of practice
  • Minimum of one year of direct nursing experience
  • Ability to travel to patient homes within the designated area
  • Reliable transportation and valid driver's license
  • Current CPR certification from an accredited organization
  • Willingness to supervise nursing staff providing in-home care to diverse patient populations
