Position Overview: This role is dedicated to delivering outpatient rehabilitation services within an in-home environment, specifically tailored for the elderly population. The service setting may include a patient's private residence, a room in a dementia-care facility, an assisted living facility, or an independent living facility. The Speech Language Pathologist will evaluate and treat patients in their most functional environment—their home—while assessing needs, developing, and implementing speech therapy interventions aimed at facilitating rehabilitation on both intermittent and maintenance levels. The Speech Language Pathologist will also identify the need for and make referrals to other disciplines to address the comprehensive needs of patients. This position involves creating individualized therapy programs designed to restore, reinforce, and enhance performance, ultimately optimizing patient independence. Additionally, the role includes instructing, educating, and training patients and caregivers in essential skills and functions to promote quality of life, independence, and productivity, while addressing language and other disorder pathologies.

Key Responsibilities:

  • Collaborate closely with the Regional Director of Therapy Operations to ensure effective execution and sustainability of Speech and Wellness Programs.
  • Conduct thorough screenings, examinations, and evaluations of patients, including comprehensive history and systems review.
  • Provide extensive SLP education to patients and caregivers in an understandable manner, planning for appropriate discharge when skilled therapeutic care is no longer medically necessary.
  • Make clinically necessary referrals to other disciplines within the care network and other agencies to address the complex and comprehensive needs of patients.
  • Consult with other members of the Therapy and Health & Wellness team to select the most suitable therapeutic regimen based on objective findings and functional needs of each patient.
  • Adapt therapy programs and/or plans of care according to individual patient needs.
  • Order necessary supplies and equipment (DME) as required.
  • Maintain, identify, and report the need for repair of equipment as necessary.
  • Deliver high-quality patient care, ensuring that patients and families receive exceptional service in a compassionate atmosphere that respects individual needs and rights.
  • Engage in departmental initiatives and demonstrate effective communication with all team members.
  • Facilitate professional growth, development, and overall enhancement of departmental performance.
  • Perform other professional duties related to patient care, development of discipline-specific therapy services, and program delivery design.
  • Apply appropriate tests, measures (FOMs), and therapeutic interventions.
  • Identify impairments, predict levels of improvement, and establish timelines for achievement; identify precautions/contraindications and design a comprehensive plan of care.
  • Adhere to continuing education requirements and maintain up-to-date licensure.
  • Services provided include, but are not limited to, therapeutic techniques in the following areas:

o Therapeutic exercises and interventions for disorders such as speech, language, social communication, cognitive communication, and swallowing.

o Articulation therapy

o Language intervention therapy

o Oral Motor therapy

o VitalStim therapy

o LSVT

o Case Management in home and community settings

Qualifications:
Minimum Requirements:

  • Valid State Speech Language Pathologist and Audiology license/certification REQUIRED to start.
  • Cardio-Pulmonary Resuscitation (CPR) certification required.

Preferred Qualifications:

  • Dementia experience.
  • Interest and/or experience in lymphedema.
  • At least 1 year of experience working with geriatric patients.

Physical Requirements: Standing (1/3-2/3), walking (1/3-2/3), using hands to finger, handle or feel (over 2/3), reaching with hands and arms (over 2/3), climbing and balancing (under 1/3), stooping, kneeling (under 1/3), talking or hearing (over 2/3), tasting or smelling (under 1/3). Lifting requirements include up to 10 pounds (1/3 - 2/3), up to 25 pounds (under 1/3), up to 50 pounds (under 1/3), up to 100 pounds (under 1/3), and more than 100 pounds (none).
