What Sets Us Apart
Position Overview: This role involves delivering outpatient rehabilitation services within the comfort of patients' homes, primarily focusing on the older adult demographic. The service environment may include private residences, dementia-care facilities, assisted living, or independent living settings. This position will assess, evaluate, and implement speech therapy interventions tailored to facilitate rehabilitation on both intermittent and maintenance levels. The Speech Language Pathologist will recognize the need for and coordinate referrals to other healthcare professionals to address the comprehensive needs of patients. Additionally, the Speech Language Pathologist will create and execute personalized therapy programs aimed at restoring, enhancing, and optimizing patient independence. Furthermore, this role includes instructing and training patients and caregivers in essential skills to promote quality of life and mitigate language and communication disorders.
Key Responsibilities:
- Collaborate with the Regional Director of Therapy Operations to ensure effective implementation and oversight of Speech and Wellness Programs.
- Conduct comprehensive screenings, evaluations, and patient assessments, including detailed history and systems review.
- Provide thorough education on speech-language pathology to patients and caregivers in an accessible manner, planning for discharge when skilled therapeutic care is no longer medically necessary.
- Make clinically appropriate referrals to other disciplines within the care network and external agencies to address complex patient needs.
- Engage with other members of the Therapy and Health & Wellness teams to determine the most suitable therapeutic regimen based on objective findings and patient capabilities.
- Tailor therapy programs and care plans to meet the unique needs of each patient.
- Order necessary supplies and durable medical equipment (DME) as required.
- Identify and report equipment repair needs as necessary.
- Deliver high-quality patient care, ensuring that patients and families receive compassionate service that respects individual needs and rights.
- Participate in departmental initiatives and maintain effective communication with all team members.
- Support professional growth and development while enhancing departmental performance.
- Perform additional professional duties related to patient care, therapy service development, and program delivery design.
- Utilize appropriate assessments and therapeutic interventions.
- Identify impairments, predict potential improvements, and establish care plans, including precautions and contraindications.
- Adhere to continuing education requirements and maintain current licensure.
- Services provided include, but are not limited to:
o Therapeutic exercises and interventions for disorders such as speech, language, social communication, cognitive communication, and swallowing.
o Articulation therapy
o Language intervention therapy
o Oral motor therapy
o VitalStim therapy
o LSVT
o Case management in home and community settings
Qualifications:
Minimum Requirements:
- Valid State Speech Language Pathologist and Audiology license/certification REQUIRED to start.
- Cardio-Pulmonary Resuscitation (CPR) certification required.
Preferred Qualifications:
- Experience with dementia patients.
- Interest or experience in lymphedema.
- At least one year of experience working with geriatric populations.
