Hospice Clinical Manager

At Liberty Hospice, we are seeking an experienced Hospice Clinical Manager - RN to join our team. As a key member of our leadership team, you will be responsible for coordinating case management of all patient services, supervising assigned care staff, and ensuring the efficient and effective utilization of existing staffing resources.
Key Responsibilities:
- Coordinate case management of all patient services, including Oasis care planning and assigning care staff.
- Supervise assigned care staff, providing guidance and support as needed.
- Ensure timely and accurate documentation of patient care and staff performance.
- Assist the administrator as necessary, providing input on operational and clinical matters.
- Provide direct patient care as necessary, including case management and cost-effective hospice services.
Requirements:
- Current RN licensure in North Carolina and successful completion of nursing education from an approved school of nursing.
- Minimum three years of clinical nursing experience, with a focus on hospice care.
- In-depth knowledge of hospice/homecare regulations and standards.
- Strong critical thinking and communication skills, with the ability to work independently and as part of a team.
- Ability to use a personal computer and have a working knowledge of various computer applications.
- CPR certification.
