Liberty Health is seeking an experienced Hospice Clinical Manager to join our team. As a Hospice Clinical Manager, you will be responsible for coordinating case management of all patient services, supervising assigned care staff, and assisting the administrator as necessary.
Key Responsibilities:
- Coordinate case management of all patient services, ensuring timely and effective delivery of care.
- Supervise assigned care staff, providing guidance and support to ensure high-quality patient care.
- Assist the administrator as necessary, providing input on operational and clinical matters.
- Provide direct patient care as necessary, including case management, Oasis care planning, and assigning care staff.
- Ensure the efficient and effective utilization of existing staffing resources.
Requirements:
- Current RN licensure in North Carolina and successful completion of nursing education from an approved school of nursing.
- Minimum three years of clinical nursing experience, with a focus on hospice care.
- In-depth knowledge of hospice/homecare regulations.
- Ability to independently organize work, provide appropriate case management, and timely and accurate documentation of care.
- Strong critical thinking and communication (oral and written) skills.
- Ability to use a personal computer and have a working knowledge of a variety of computer applications.
- CPR certified.
