Key Responsibilities- Manage a designated client caseload, ensuring consistent communication to evaluate and monitor client progress.
- Assess client strengths, needs, and challenges while working to eliminate obstacles to achieving their goals.
- Utilize a strengths-based and trauma-informed methodology to collaborate effectively with clients and both internal and external team members, fostering support in their recovery journey.
- Facilitate community integration, transportation assistance, and the development of essential life skills.
- Engage in goal setting and treatment planning with clients.
- Connect clients with community resources and provide necessary referrals.
- Deliver supportive services that promote client independence.
- Oversee medication management and intervene during crises as needed.
- Maintain accurate and timely documentation of all client interactions and activities.
- Adhere to all relevant internal and external policies, procedures, regulations, and standards, including the agency's Quality Assurance Plan and State Licensing requirements.
- Participate in required meetings, staff development, and training sessions.
- Associate or Bachelor's degree in Social Work, Human Services, or a related field, with a minimum of one year of experience in an alcohol and other drug (AOD) or mental health environment.
- Possession of an active CDCA or LCDCII/LCDCIII/LICDC license from the Ohio Chemical Dependency Professionals Board.
- Experience in case management and familiarity with community resources.
- Strong interpersonal and engagement capabilities.
- Excellent organizational and time-management skills.
- Proficient written and verbal communication abilities.
- Competence in computer skills, particularly with electronic medical records (EMR) documentation.
- Commitment to supporting the agency's mission and philosophy, with an emphasis on cultural sensitivity and workplace harmony.
- Dedication to providing exceptional customer service.
Work Environment: This position requires in-person engagement.
