We are seeking a highly skilled and compassionate Care Coordinator to join our team at BCFS Health and Human Services. As a Care Coordinator, you will play a critical role in assessing the needs of minor clients and coordinating responses to ensure their safety and well-being.
Key Responsibilities- Participate in an on-call rotation schedule for the Care Coordination Hotline, responding to crisis situations and documenting information as necessary.
- Document and maintain all data for grant metrics, including report/notification information, demographics, services provision, and investigative steps.
- Assess whether the identification or recovery of a minor meets agreed-upon protocols to launch coordinated services.
- Dispatch advocate response, as determined by working CCT protocol.
- Set up Rapid Response Team meetings with partners within 48 hours of notification of the identified or recovered minor.
- Maintain contact with law enforcement and CPS investigators to facilitate initiation of joint investigations.
- Attend MDT meetings, as needed, to represent clients and the Care Coordination Team.
- Develop needs associated with Care Coordination Team meetings, including agenda, locations, and attendee notification.
- Implement BCFS HHS safety protocols in case of an emergency.
- Participate in weekly internal staffing, supervisions, Care Coordination meetings, and community meetings.
- Create an information tracking system to track requested volunteer information and support quarterly report development.
- Establish subject matter expertise on community resources.
- Maintain confidentiality in all areas of the service population and program operations.
- Maintain BCFS HHS professional and ethical standards of conduct, including demonstrating respect for agency staff, the service population, and community members.
- Pass a pre-employment drug screen and random drug screens, and maintain emotional control and professional composure.
- Provide proof of work eligibility status upon request.
- Pass pre-employment and biennial criminal background checks.
- Demonstrate the ability to respond sensitively and competently to the service population's cultural and socio-economic characteristics.
- Communicate effectively in writing and verbally in English; bilingual ability in Spanish is strongly preferred.
- Work in a fast-paced environment and maintain emotional control and professional composure.
- Maintain computer literacy required to meet the responsibilities of the position.
- Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
- Actively support supervisees in professional development.
- Demonstrate a working knowledge of all Human Trafficking Interdiction Division policies and procedures.
