We are seeking an experienced Internal Medicine Physician to join our team at Yale New Haven Health. As a primary care physician, you will be responsible for providing high-quality patient care and leading an interdisciplinary team of caregivers and administrative support staff.
Responsibilities:- Examine patients, obtain medical histories, and treat acute and chronic illnesses
- Order, perform, and interpret diagnostic tests
- Involve patients and their families in care and treatment decisions
- Create treatment plans and complete orders in patient charts
- Collaborate on improvements in care delivery and service with staff, other physicians, and management
- Initiate appropriate specialty referrals
- Provide clinical information to the interdisciplinary team in a clear and timely manner
- Ensure proper patient documentation is timely, accurate, and complete
- Work with practice management to ensure proper compliance with billing, coding, and CPT code assignment
- Achieve and maintain optimal patient access
- Provide interdepartmental medical consultation as requested
- Participate in the development of medical protocols, standard procedures, and management of patient-care-related activities
- Provide on-call coverage
- Give feedback to and accept feedback from all staff in a respectful manner
- Act in a positive and professional manner, complying with practice policies, procedures, and federal and state laws
- Perform other duties and responsibilities as assigned
- MD or DO degree with ABIM-recognized internal medicine residency (three years minimum) or ABFM-recognized family medicine residency (three years minimum)
- Valid American Board certification in either internal medicine or family medicine and renewed (if needed) prior to expiration
- Valid and unrestricted Connecticut medical license
- BLS certification
- Valid DEA registration and Controlled Substance Certificate
