The Lead Care Coordinator is a key member of the multidisciplinary team at Northern Arizona Healthcare, responsible for facilitating patient transitions and ensuring seamless care coordination. This role requires strong leadership and communication skills, with a focus on identifying and addressing complex patient needs.
Key Responsibilities- Care Coordination and Transition Planning
- Proactively assess patient acuity and transitional needs, working closely with the Care Management team to address barriers and obtain necessary resources.
- Partner with the interdisciplinary team to facilitate patient placement in appropriate care settings, including rehab, SNF, sub-acute, assisted living, and home health care.
- Identify and arrange consultative services to enhance patient transitional plans.
- Encourage patient and family participation in care team decisions.
- Collaborate with post-acute services, care managers, and PCPs to ensure smooth transitions back to the home environment.
- People Management
- Assist in adjusting daily assignments based on staffing and caseload acuity.
- Demonstrate knowledge of NAH policies and procedures, ensuring compliance with best practices.
- Establish and maintain mutual trust and respect with staff, promoting a motivated and empowered work environment.
- Support department communication lines through huddles, department meetings, and other means of fostering employee engagement.
- Work with employees to facilitate skill, educational, and performance goals.
- Address workflow issues, ensure incidents are documented, and corrective steps are taken.
- Resource Management
- Evaluate the appropriateness of care delivery in the inpatient setting, communicating discrepancies with the medical team.
- Facilitate care management in the outpatient setting.
- Take a leadership role in identifying opportunities to reduce risk, both financial and clinical, through analysis and resource consumption outcomes.
- Assertively manage resource utilization while navigating patient movement along the continuum of care.
- Compliance and Safety
- Stay current with state and federal regulations, as well as company policies impacting the role.
- Ensure all certifications and licenses are up-to-date and valid prior to expiration dates.
- Complete mandatory company modules and job-specific training in a timely manner.
- Report safety-related incidents through the Midas/RDE tool, attend safety training programs, and perform work in a safe manner.
