Job Title: Visitation Coach

Diakon Lutheran Social Ministries is a leading non-profit organization in Pennsylvania and Maryland, providing exceptional services to the community. We are seeking a highly skilled Visitation Coach to join our team.

Job Summary:

The Visitation Coach will be responsible for employing a range of social work interventions with client families to facilitate optimal environments for visitation. This includes making initial and ongoing case assessments for family needs and strengths with parents, children, and resource parents.

Key Responsibilities:
  • Assist parents in identifying and completing goals needed for reunification as outlined by their Family Service Plans.
  • Makes regular contact with clients as well as participates in group functions for children, biological, and foster families as required.
  • Collaborate with other professionals to ensure comprehensive support for client families.
Requirements:
  • A BA or BS degree in social work, BSW, normally required.
  • A minimum of two (2) years' experience in a related child welfare or human service position normally required.
  • Experience in social work or case management is essential.
