Job Title: Licensed Practical Nurse

As a key member of our nursing team, you will provide compassionate care and support to clients in residential care facilities, behavioral health, and substance use disorder treatment programs.

Key Responsibilities:
  • Administer medications to clients via various methods, ensuring proper dosage and adherence to protocols.
  • Conduct comprehensive nursing assessments, including physical, mental health, and psychiatric evaluations.
  • Maintain accurate and timely documentation of medication administration, assessments, and client interactions.
  • Monitor medication inventories, ensuring adequate stock and timely refills for residential clients.
  • Communicate effectively with clients, addressing medication queries and coordinating refill requests with providers.
  • Verify client data, medical history, and visit purposes, facilitating seamless care delivery.
  • Participate in nursing-led educational groups and contribute to client safety planning.
  • Assist with general lab duties and phlebotomy as needed.
  • Serve as a resource for nursing and medical issues, promoting client hygiene and infection control measures.
  • Actively participate in required meetings and perform other assigned duties as delegated by nursing leadership.
Requirements:
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
  • Graduation from an accredited practical nurse program.
  • Required License/Certification: Licensed Practical Nurse (LPN).
  • Additional Qualifications:
    • Successful completion of background check including criminal record, driving record, and abuse/neglect.
    • Valid driver's license in state of residency with an acceptable driving record.
    • Strong communication skills, both verbal and written, with the ability to produce clear and concise reports.
    • Proficient in accurately entering, transcribing, recording, and storing information in various formats.
    • Skilled at reading and interpreting various documents, including laboratory results and electronic health records.
    • Competent in basic mathematical calculations.
    • Possesses strong problem-solving abilities and adaptability.
    • Demonstrates a willingness to learn, grow, and work collaboratively with diverse perspectives.
