Job Summary

We are seeking a skilled Registered Nurse to join our clinical team as a Care Coordinator. In this role, you will provide high-quality patient care and coordination services to support our healthcare team.

Key Responsibilities
  • Manage patient care and follow-up tasks within the electronic health record (EHR)
  • Coordinate care for upcoming patient appointments and ensure continuity of care
  • Assist with non-medication prior authorizations and diagnostic testing
  • Participate in daily care team huddles to facilitate communication
Requirements
  • Valid license as a Registered Nurse in the state where nursing practice will occur
  • Two years of nursing experience in an outpatient clinic, preferred
  • Basic Life Support (BLS) certification through American Heart Association within six months from date of hire

Working Conditions

This is a remote position. The employee is regularly required to sit, use hands and fingers to operate a computer keyboard, handle, or feel, reach with hands and arms, and talk or hear. The employee is occasionally required to stand. The employee may occasionally lift and/or move up to 10 pounds.
