Job Summary

Community Health Center of Southeast Kansas is seeking a compassionate and skilled Registered Nurse to join our Hospice health team. As a Hospice Care Nurse, you will provide high-quality nursing care to patients in their homes, ensuring they receive the medical attention they need to live comfortably and with dignity.

Key Responsibilities

Conduct thorough assessments of patients' health needs and develop personalized care plans
Provide skilled nursing care, including medication administration, treatments, and procedures, in accordance with established policies and procedures
Collaborate with the healthcare team to ensure seamless care coordination and communication
Direct and supervise Home Health Aides, as needed
Maintain accurate and detailed records of patient care and progress

Requirements

Valid Kansas license as a Registered Nurse or multistate license in the nurse's state of residence
One year of experience as a Registered Nurse in acute care, long-term care, or similar field
BLS Certification
Current Auto Insurance and valid driver's license, as travel is required

Work Environment

Fast-paced and dynamic work environment
Regularly required to bend, stoop, squat, kneel, stand, and walk

Computer work is frequent and regular
May involve exposure to potentially dangerous materials, infectious diseases, and situations that require following extensive safety precautions
