Franciscan Villa has an exciting opportunity for an RN MDS Coordinator.
The RN MDS Coordinator plays a key role in ensuring the accuracy of resident assessments and the optimization of care plans. Key responsibilities include:
- Leading and coordinating the development and completion of resident assessments (MDS) in compliance with current regulations and guidelines.
- Ensuring the MDS process is completed accurately and within required timeframes.
- Collaborating with the interdisciplinary team to create personalized care plans for residents.
- Continuously monitoring and documenting residents' progress and responses to care plans.
- Actively participating in the Quality Assurance and Performance Improvement (QAPI) process to enhance care quality.
- Communicating effectively with residents, families, and healthcare providers to ensure seamless care coordination.
- Staying informed on MDS regulations and guidelines to ensure compliance with industry standards.
This role is essential in maintaining high standards of care and regulatory compliance within the facility.

Requirements:
- Current RN (Registered Nurse) license

- Ability to work independently and collaboratively within a team
