Position Summary:The Clinical Leader – RN Case Manager assesses the needs of the department and collaborates with health team members to coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care. Under the supervision of the Clinical Manager/Director, this position supports the RN Case Managers in the department according to policies and procedures.Assists with the daily operational aspects of the department with respect to facilitation of work priorities.
