Our Ohio-based management team, through our locally-based clinicians, can respond to the needs of our patients in a timely and appropriate manner because we are local.We are also passionate about providing our employees the right tools to do their jobs effectively and provide the best hospice care in the Cincinnati, OH area. We don’t require high caseloads or unmanageable quotas, which gives our team more time with each of their patients. Our top priority is our patients and the people serving them.What We offer:Great culture and team atmosphereFlexible HoursMileage reimbursementEmployee Referral Program Employee Discount ProgramsWhat You’ll Do:Performs spiritual bereavement assessments of patients and families/caregivers in the hospice program in a timely manner and ensure that identified needs are addressed in the patient’s plan of careProvides direct spiritual care and bereavement counseling to patients and families/caregiversDemonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assists team members as needed to ensure that this level of service is consistently providedProvides accurate and timely documentation of patient services to reflect the plan of careAttends group meetings to facilitate coordination of care and discussion of interdisciplinary group involvementProvides consultation, education, and support to the interdisciplinary group on spiritual care and bereavement counselingParticipates in Continuous Care and on call, if necessaryAssists in supervision of spiritual care volunteers when assigned to patient’s families/caregivers as neededAssumes responsibility in directing the development and implementation of the bereavement plan of care and the frequency of service delivery; make available bereavement services for up to one year following the death of the patientQualifications:Graduate of an accredited seminary or school of theology and/or a certificate of ordination or commission from a religious faith group; completion of Clinical Pastoral Education (CPE) preferredPrevious hospice experience working with death and dying strongly preferredComfortable in an interfaith setting and able to accept different lifestyles, cultures, beliefs, and valuesValid driver’s license and proof of insurance is requiredText 7047 to (937)863-9487 to apply#ACHOSEmployee wellbeing is top priority at Addus Homecare, and we're thrilled to announce our recognition as the top healthcare company on Indeed's 2024 Top 100 Work Wellbeing Index.
