About the Department:This department supports the collaboration efforts through patient and provider education, patient navigation, referring provider coordination, supporting consistent processes, and enabling strong and routine communication between East and West. This position would be employed by the CH-West Transplant Program but would be deployed at and embedded in the CH-East Royal Oak pulmonary practice. This individual would provide increased insight and enable increased connection to the East side providers and referring hospitals.About Royal Oak HospitalRecognized by U.S. News & World Report as the #2 hospital in Michigan and one of the top hospitals in the country. The advanced capabilities and clinical expertise within this center have earned it Magnet designation four consecutive times and has the distinct honor of housing Michigan’s first Diagnostic Imaging Center of Excellence.Scope of workCoordinates advanced lung disease and transplantation of all recipients. Provides follow-up and care across the continuum from advanced lung disease to pre-transplant, during transplant and post transplant. Collaborates with members of the health care team to enhance patient care by developing, supporting and educating nursing staff. Makes clinical judgments based upon a synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems. An integral member of a larger interdisciplinary team contributing to excellence in patient care, research and teaching.Facilitates advanced nursing care for patients before and after transplantation along the continuum of outpatient to inpatient as required.Coordinates evaluation process for pre-transplant patients and living donors (as applicable) to ensure effective use of hospital and/or clinical resources.Assesses post-transplant and post donation (as appropriate) organ function and general health status and collaboratively determines need for additional assessment or intervention.Participates in the development and implementation of protocol and standards of care for the transplant patient in collaboration with the multidisciplinary team.Provides on-call coverage including coordination of activities prior to transplant, the transplantation process when a donor is available (24/7), communication between physicians, local organ procurement organization, patients/families and the medical team involved, blood bank in collaboration with the physician on-call.Completes necessary documentation according to the United Network of Organ Sharing (UNOS) guidelines including listing, status changes and post-transplant/post-donation (as applicable) follow-up information.Communicates with multidisciplinary team members and patients and families in a timely manner in regard to patient care/issues and to develop discharge plans for patients pre-transplant or post-transplant including referrals for necessary discharge care.
