We are seeking a dedicated and compassionate PRN Hospice Licensed Practical Nurse (LPN) to join our hospice care team. You will work under the supervision of a Registered Nurse (RN) to deliver personalized care and ensure the comfort and dignity of patients during their end-of-life journey. As a Hospice Licensed Practical Nurse, here’s a big-picture view of what you’ll do:
- Provide home-based care to patients with a prognosis of six months or less to live, focused on quality of life, under the supervision of an RN
- Work as part of an interdisciplinary team comprised of a physician, RN, CNA, social worker, chaplain and volunteers to carry out the patient’s plan of care
- Perform nursing services which are planned, delegated, and supervised by the registered nurse
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
- Take a holistic approach to care that engages the patient’s mind, body, spirit and family
A few must-haves for Hospice Licensed Practical Nurses:
- Graduate of an accredited practical nursing program and active LPN license in Louisiana
- Minimum of 1 year of hospice LPN experience
- CPR certification
- Valid Louisiana driver’s license, auto insurance and transportation
- Knowledge of state and federal home health and hospice regulations
- Clinical proficiency in nursing, strong communication skills and compassion
