How You Will Make a Difference
As we expand our services in new and existing markets, we're looking for a Care Transition Navigator who shares our values of being Trustworthy, Capable, Compassionate, Proactive, and most importantly, Called to serve patients in their home. The Care Transition Navigator is the face of VitalCaring when interacting with referral sources. In this role, you orchestrate the perfect balance of patient care and operational efficiency, ensuring the patient experiences a smooth transition home. As a result, your clients view VitalCaring as their preferred home health partner that provides compassionate, personalized home health care to their patients.
As a Care Transition Navigator, you will:
