Provides general medication assessment and management based on current nationally recognized guidelines. Assess and manage medication therapy for comorbidities which could increase the patient's cardiovascular risk to include: Lipids, Blood pressure, and Renal protection. Provides educational interventions based on the AADE7 recommendations: Healthy Eating, Being Active, BG Monitoring. Referral to appropriate specialists and ancillary services, including but not limited to nephrology, pulmonology, psychiatry, ophthalmology, podiatrist, wound care, and for vaccinations. Develop and communicate an individualized follow-up plan with each patient. Document each session via EMR in a timely manner. Communicate all plans to cardiologist, endocrinologist, primary care provider, or other referring physician via EMR. Evaluates effectiveness of management and educational services by measuring attainment of patient-specific goals. Participate in the development of training materials. Contribute to, and participate in, a continuous quality improvement process. Qualifications Education: Requires a bachelor or Pharm D. degree from an accredited school of Pharmacy. American Society of Health System Pharmacist Pharmacy Practice Residency preferred Experience: Priority will be given to a PharmD who is a certified diabetes educator, or has clinical diabetes medication management experience. Minimum 6 month experience providing diabetes management and 15 contact hours within the past year in diabetes clinical and educational subjects is preferred.
