Wood County Hospital is accepting applications for Nurse Care Manager. This position is responsible for effectively delivering care delivery with the goal of increasing patient quality and outcomes while reducing their spend and utilization. Coordinates care with the Primary Care Physician and Advanced Nurse Practitioner (PCP) and functions as part of an interdisciplinary team to guide high risk patients across care delivery sites, including inpatient, ambulatory and post-acute care settings. Ensures continuity of care through defined, evidence -based methods, including, but not limited to, medication reconciliation, self-management planning, engagement of family and care giver, health education, and referrals. Collaborates with other care team members to address gaps in care.

Job Duties:

  • Performs an assessment of the patient's current health status, gaps in care, utilization patterns, and barriers to care, to develop a self-management plan of care in conjunction with the patient/family/caregiver, provider, and interdisciplinary team.
  • Serves as a bridge and consistent point of contact across clinical settings by working in collaboration with the provider, office staff, community resources, inpatient and ambulatory healthcare staff to facilitate continuity and care coordination across the continuum of care.
  • Performs home, hospital, and community setting patient visits as necessary.
  • Aids with clinical decisions related to transitions of care and appropriate community resources to improve patient outcomes.
  • Documents in the medical record and designated case management tool accurately reflecting collaborative care planning, interventions, and evaluation against defined targets and goals.
  • Maintains a high regard for patient advocacy, patient choice for care and treatment services, and patient satisfaction.
  • Complies with all practice and department policies and procedures; monitors/addresses staff compliance with those related to care management.
  • Supports the mission, vision, and values of the organization.
  • Assumes responsibility and accountability for individual knowledge, personal/professional growth and development, skills, performance, and behavior in accordance with hospital, division, and department standards of care and policies and procedures.
  • Other duties as assigned.
Shift Details:

Hours: Full Time - 40 hours per week

8:00 am- 4:30 pm

Minimum Qualifications:

Minimum Education:
  • Registered Nurse (RN) with an active license in the state of Ohio.
  • BSN/ADN degree or professional certification/license
Minimum Experience:
  • 2 years of experience
  • 2 years case management preferred
