Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient's recovery from illness, injury or surgery at home.

  • You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education
  • You will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients
  • You will review patient insurance information and coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers
  • Current NYS RN (Registered Nurse) license and registration required
  • Bachelor's degree in nursing preferred; Associate's degree required
  • Minimum of one year acute medical-surgical nursing experience required
  • Certified Home Health Agency (CHHA) experience preferred
  • Bilingual English and Spanish or Mandarin or Cantonese preferred
