The Social Worker (SW) staff augmentation role will fill in for short-term staffing needs with Optum care management teams as well as provide consultative support to the front-line care management team members as appropriate. During a short-term clinical assignment, this SW role will provide comprehensive social work services to individuals and families in various client locations, addressing the organization's unique patient population needs. This role will be responsible and accountable for coordination of needed patient and family services as part of the interdisciplinary team, providing a clinical and psychosocial approach over the entire continuum of care. Discharge planning will begin at the time of (or prior to) admission and will be reassessed throughout the course of the hospitalization. The SW role will provide direct social work services to patients and/or families for assessment of discharge needs, ongoing support, and community resource referrals. This individual will collaborate with other members of the community health care team to assure success in the home environment and avoid readmission to the hospital. This social work role may include the provision of crisis intervention strategies which requires a comprehensive understanding of mental health diagnoses as well as brief solution-focused counseling skills. Relationship building skills with both hospital staff and community providers will be critical to effective discharge planning.

In addition, as required, this individual will provide consulting services to frontline leaders and team members regarding care management redesign activities, this individual will work alongside client teams to solve complex business problems, improve performance, and execute high priority initiatives by conducting and/or interpreting analyses, creating deliverables, and helping drive project execution for Optum's transformational clients.

The ideal candidate for this role must be willing to travel 100% and be passionate about driving improvements in performance, effective at working in a fast-paced, high-energy environment, and confident in their interactions with senior leaders, providers, and business partners.

Primary Responsibilities:

  • Professional Accountabilities
    • Adheres to name badge/dress code compliance as well as any other organizational requirements
    • Effectively problem-solves and actively pursues resolution of complex problems
    • Directly and effectively communicates with staff, physicians, patients, and families at all times
    • Consistently models leadership behavior and demonstrates all of Optum's core values
    • Coordinates identified patient care discharge solutions to achieve desired quality outcomes and identifies and intervenes with inappropriate resource utilization
    • Remains flexible to meet the needs of the hospital, which may include floating to other departments to assist as patient needs fluctuate
    • The SW provides skilled interventions for:
      • Patient/family support as well as resolution of complex patient/family situations
      • Patient advocacy in an interdisciplinary setting as necessitated by life-changing events (e.g., new diagnosis, abuse or neglect, loss of independence, acute grief, and loss reaction, etc.)
      • Identification of risk and safety factors as well as other psychosocial issues
    • Continuum of Care Planning
      • Assessment of the need for post-hospital services and determination of an appropriate discharge plan for all post-acute care cases, alcohol/substance abuse, complex readmission cases, and homelessness cases
      • Educates patient/family as to options/choices within the level of care determined to be appropriate and initiates all necessary documentation, orders, and paperwork to facilitate timely discharge
      • Emphasizes education and collaboration with physicians, family members, nursing staff, therapists, and case managers from contracted payors when appropriate to determine plan of maximum benefit to the patient. Involves community providers in the treatment plan as early as possible to promote continuity and collaboration
      • Creates warm handoff to post-acute providers to ensure that patient needs will be met, thus decreasing the need for readmission to the hospital
    • Risk Management
      • Interfaces with department directors, Risk Management, and patient representatives to identify potential QA or risk issues. Performs any necessary investigation, documentation, and follow-up as required
      • Participates in departmental process improvement projects
      • Functionally coordinates and plans discharge for all age groups, including but not limited to the unborn child through geriatric age groups
      • Complies with all regulatory requirements, including state and federal standards
    • Consultative Services
      • Supports front line redesign for care management transformation in conjunction with operations and transformational leaders and team
      • Synthesizes findings and summarizes a broad range of data inputs into outputs that clearly communicate data findings and insights
      • Defines, develops and documents business requirements to ensure clients' needs are captured and delivered
      • Creates written and oral communication artifacts or deliverables that effectively summarize findings, support fact-based recommendations, and provide appropriate detail to substantiate conclusions
      • Provides any of the following:
        • Monitor operational or service line agreement metrics for improvement
        • Partner with Optum Education team to develop standard work tools, job aides, or guidelines
        • Provide education to front line care management team members
        • Coach front line team members on new processes or workflows
        • Support implementation of care management processes, such as interdisciplinary rounds or long length of stay rounds observations and auditing
        • Perform chart reviews to assess care management team performance

Required Qualifications:

  • 3+ years of experience in social work in an acute care hospital setting
  • Experience working with culturally diverse populations and familiarity with various cultural norms and practices
  • Demonstrated understanding and appreciation of cultural diversity, with the ability to work effectively with individuals from diverse backgrounds and communities
  • Proficiency in case management software and documentation tools
  • Proven ability to adhere to the National Association of Social Workers (NASW) Code of Ethics and commitment to maintaining the highest professional standards in social work practice
  • Proven solid interpersonal and communication skills, with the ability to build rapport and trust with diverse individuals and communities
  • Proven exceptional problem-solving and decision-making abilities, with a client-centered approach to service delivery
  • Ability to travel 100% of the time, often to remote or underserved areas, and adapt to varying work environments and client needs

Preferred Qualifications:

  • State licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW), as applicable in the state of service
  • Certifications or specialized training in crisis intervention, trauma-informed care, or related areas
  • Clinical knowledge of Labor and Delivery, Mental Health, Neonatal, Medical Surgical, and Oncology patients
