Your Responsibilities:
- Conduct thorough physical assessments for patients of all ages, including children from age 3 and up.
- Provide ongoing management of chronic conditions such as hypertension, diabetes, and asthma.
- Administer annual wellness visits, including vaccinations and preventive screenings for both adults and children.
- Order and interpret diagnostic tests essential for assessing patients' clinical issues.
- Collaborate with healthcare professionals to formulate comprehensive care plans.
- Prescribe medications, recommend treatments, and provide clear post-visit instructions.
- Refer patients to specialists and other healthcare resources as necessary.
- Educate patients and their families on health maintenance and preventive care strategies.
- Promote patient engagement, health literacy, and shared decision-making.
- Participate in quality improvement initiatives and value-based care programs.
- Focus on delivering patient-centered care to foster strong relationships with your patients.
Experience and Education Requirements:
- Two or more years of experience as a Nurse Practitioner or Physician Assistant in a direct primary care setting.
- Proficiency in providing comprehensive primary care, emphasizing chronic care management and preventive health.
- Strong understanding of Patient-Centered Medical Home (PCMH) principles and value-based care.
- Willingness to perform pediatric exams and see younger patients.
