Your Responsibilities:

  • Conduct thorough physical assessments for patients of all ages, including children from age 3 and up.
  • Provide ongoing management of chronic conditions such as hypertension, diabetes, and asthma.
  • Administer annual wellness visits, including vaccinations and preventive screenings for both adults and children.
  • Order and interpret diagnostic tests essential for assessing patients' clinical issues.
  • Collaborate with healthcare professionals to formulate comprehensive care plans.
  • Prescribe medications, recommend treatments, and provide clear post-visit instructions.
  • Refer patients to specialists and other healthcare resources as necessary.
  • Educate patients and their families on health maintenance and preventive care strategies.
  • Promote patient engagement, health literacy, and shared decision-making.
  • Participate in quality improvement initiatives and value-based care programs.
  • Focus on delivering patient-centered care to foster strong relationships with your patients.

Experience and Education Requirements:

  • Two or more years of experience as a Nurse Practitioner or Physician Assistant in a direct primary care setting.
  • Proficiency in providing comprehensive primary care, emphasizing chronic care management and preventive health.
  • Strong understanding of Patient-Centered Medical Home (PCMH) principles and value-based care.
  • Willingness to perform pediatric exams and see younger patients.
