Shelter Case Manager

Case Managers help our clients and guests gain access to resources and the support they need to become contributing members in the community. Responsibilities:

Provide supportive counseling/case management services and create access to safe, decent and affordable housing opportunities. Case managers facilitate skills building, service coordination/advocacy, linkage to community-based resources and crisis support for IHS’s homeless guests and clients. Work collaboratively with other community providers, IHS program staff and treatment teams to ensure each guest/client meets their desired goals.

Essential Functions

  1. Demonstrate excellent customer service consistent with the standards contained in the Vision, Mission, and Values of the organization.
  2. Conduct guest intakes and assessments and identify, develop and meet desired outcomes of guest by maintaining a warm and welcoming attitude throughout their stay; may or may not involve family and/or significant others based on the guest’s desire.
  3. Advocate on behalf of IHS guests; develop and ensure that each client has a current individual service plan as well as develop a transition/discharge plan for each guest no longer meeting the criteria for continuing care.
  4. Refer guests to community resources; provide guests with access to healthcare, substance abuse treatment services and/or support systems within the community.
  5. Provide supportive counseling or problem-solving activities to assist guests in developing and maintaining pro-social skills necessary to manage or improve mental or physical health.
  6. Respect and maintain the confidentiality of the guests served; ensure appropriate standards for release of protected information and confidentiality laws are followed as required by both state and federal agencies.
  7. Responsible for the integrity of the clinical record and files including the completion and maintenance of all required clinical documentation according to IHS standards; accurately enter data into systems of record, using the VI-SPDAT as part of Coordinated Entry.
Position Type/Expected Hours of Work

Regular Full-time classification. General business hours, Monday through Friday, including some evenings, weekends and holidays. Hours and days may vary based on the needs of the organization.

Required Education & Experience
  1. High School diploma or GED equivalent
  2. Some experience in case management or facilitating any supportive services and/or working with homeless or other disenfranchised populations.
Preferred Education & Experience
  1. Bachelor’s degree in Psychology, Human Services or related field
  2. One to two years case management experience in nonprofit setting serving homeless individuals
  3. Experience navigating healthcare, human services and substance abuse systems of care
  4. CPI certified or able to pass at IHS; CPR and First Aid certified or able to certify through IHS
  5. Valid Driver’s License and clean traffic abstract
